00:01me again uh we've had a lovely
00:03conversation last year when you guys
00:05launched smart MDI uh system for the
00:09diabetes community in Europe uh thanks
00:11for doing a followup i' would love to
00:13hear your experience one year in how the
00:17how has the European Community responded
00:18to the smart MDI system and what's been
00:20like yeah it's been really great so we
00:23initially launched and currently we're
00:25in 13 different countries and this was
00:28just about a year ago and so since then
00:30we've actually just launched our smart
00:33MDI system with the simpa CGM so I have
00:37it with me here but we have our new
00:39sensor so you'll notice um completely
00:42different form factor and a new easy
00:46insertion process so we say it's really
00:48two steps the first is just
00:52uncapping the product and then pushing
00:57and placing it on there's no over taper
01:00ired now we have our sensor and our
01:02transmitter in one it's fully disposable
01:05after 7 days no finger sticks required
01:09so we're really excited to continue to
01:11advance and evolve our system first with
01:14the new CGM and then also we've brought
01:17two new smarter um dosing notifications
01:22is what I like to call them right we're
01:24just continuing to advance the system
01:26and I think it's really exciting mmdi is
01:30um technology type really for the
01:32diabetes community in Europe we've had
01:35uh pumps and hybrid clo for a long time
01:37we had kind of pens and syringes that
01:40that that kind of connection of both
01:42that Gap in the middle um for those who
01:45are still unsure what smart MDI actually
01:48is yeah what's what's a 45 second Pitch
01:51of it sure so I always talk about that
01:54there's been I I mean Pump therapy is
01:57amazing it's the standard but we've
01:59watched pump therapy evolve and grow so
02:02much and I say that us people on MDI
02:05therapy have been kind of left in the
02:07Dark Ages Left Behind of like pencil and
02:10paper and no way of knowing what we took
02:14when we took it how to calculate our
02:16Doses and so I really see this system as
02:21elevating um Bridging the Gap in the
02:23technology and helping to get people
02:25that either are on MDI because of access
02:30cost or just personal preference like
02:32myself um we're really helping to
02:35elevate what's possible with MDI therapy
02:38yeah um so uh the gray pen you hold in
02:42there that works with the Lily insulin
02:45Eli Li correct and you also have a blue
02:49smart MDI pen which works with the novel
02:53in is that right exactly so you open
02:56this up and this cartridge holder gets
02:58removed and you put either the Lily or
03:02the Nova cartridge inside of it and
03:04that's the part that's um let's say
03:07disposable each time and then this part
03:10is reusable for 365 days you mentioned
03:15the new uh smart notifications yeah
03:19what's that why don't you uh give us a
03:21quick overview of that new feature yeah
03:23so this aspect I actually really really
03:26love um because when you think about CGM
03:29only or just wearing a CGM often alerts
03:33are just Guided by how quickly is your
03:36glucose rising or did you cross over a
03:38glucose threshold but when you think
03:40about diabetes management it's not just
03:43what's my glucose but it's when did I
03:45last take a dose how much insulin do I
03:47still have working in my body and so now
03:50that we have a system that brings
03:52together insulin dosing and a continuous
03:55glucose monitor you can be smarter about
03:58how we alert when we alert and really
04:01think about it in the context of
04:02diabetes management not just glucose
04:05control so we have a misos alert which
04:08is looking at a rate of change so when
04:10your glucose starts Rising quickly we
04:13check to see if there's a surrounding
04:15insulin dose and if not then we're
04:18prompting hey did you miss a dose your
04:20glucose is rising and so that's one way
04:23of kind of you know getting a glucose
04:25alert but tying it back to therapy and
04:27then giving you action through the the
04:29dose calculator of what to do now now
04:32that I missed a dose what should I take
04:34and then next we have our correct high
04:37glucose alerts so again I so I talk
04:41about this even from my own experience I
04:44have to kind of with CGM alone I would
04:47choose between maybe a lower glucose
04:50alert like at a 180 but say a threshold
04:55uh or 10 Millo and um but often what
04:59that meant is that I would get alerted
05:01often and there was nothing I could do
05:03about it um and so then sometimes
05:05patients elect or users elect to raise
05:08the the alert threshold so that they
05:10don't get interrupted as much but it
05:13doesn't mean that I don't want to do
05:15anything until my blood sugar is over a
05:18really high value right so now these
05:20alerts are smarter and thinking about
05:22not only is your glucose high but is
05:25active insulin low enough that we know
05:28additional insulin is going to be needed
05:29to get you back to Target so if you just
05:33took insulin and your glucose is high we
05:36don't need to interrupt you because we
05:38just need to wait for insulin to do its
05:40job right so those are the two no new
05:43dosting notifications that personally I
05:47think are incredible for MDI therapy
05:50because it allows for Less interruptions
05:52and then guidance on what to do next so
05:56so would you say because they sound uh
05:58very similar to War pump users
06:00especially in hybrid Clos Loop had the
06:03ability to have your 780g system for
06:06example Alo bolus when it almost detects
06:09that there's been a high glucose Spike
06:12and kind of it is is that almost the
06:14same methodology that you kind of
06:16thought this is the technology that the
06:18pump Community has been able to get now
06:21with notifications we can actually bring
06:23some of that uh proactiveness to pen
06:27Community yeah I mean so knowing how
06:30much insulin on board allows us to to
06:33know like when is action needed or when
06:36can I make a when can I take a dose is
06:39when do I not need to right and so I see
06:41these alerts I mean we've done some
06:43research we've done simulations and
06:46estimated um by using this alert instead
06:49of a traditional high alert right uh you
06:53expect maybe a reduction of 50% of your
06:57CGM alerts and alarms and you know it's
07:00not the same algorithm that we're using
07:01in the pump but it is thinking about
07:04things similarly what are the challenges
07:07sometimes patients or users forget to
07:09dose that's okay I I do that I do that
07:12this morning it happens right we're all
07:13busy we're living life sure and knowing
07:16that the system is going to help to
07:17detect it and say hey you forgot here's
07:20what here's what you need to do to you
07:21had to yeah the app updates and the new
07:26notifications you mentioned uh the the
07:30a smart MDI system uses across uh kind
07:33of Europe how can how can they get those
07:36two new notifications which just
07:37discussed the good news is it's actually
07:40really easy you go to the App Store and
07:43you download the latest version of the
07:45app y so the same pen the existing pen
07:49will continue to work with the
07:51application and then um you know as you
07:55move to the new sensor you just continue
07:57to connect it through the impan app but
08:00but really there's there not much work
08:02you just update to the latest version of
08:04the app in the app store great I just
08:06wanted to close on folks a little bit on
08:09uh the UK so um first of all uh if you
08:12could share with us the the the timings
08:15for the the updated um smart MDA system
08:19of simplura for the UK and if someone is
08:22is interested in um kind of learning
08:25more um uh who in The Diabetes Care do
08:29they need to speak to as a primary care
08:30or secondary care yeah so actually next
08:34week we'll be officially launching in
08:36the UK and so in order to get the system
08:39you'll just need to speak with your
08:41health care provider in secondary care
08:44in diabetes clinic and then we also have
08:46an option for you to shop for it on our
08:49eShop in the UK so look forward to this
08:52being available starting next week
